Christie Rehab at Bishop Gadsden is seeking a compassionate and skilled Social Worker to lead the delivery of social and emotional health services for our residents and patients. In this dynamic role, you’ll build trusted relationships, provide counseling and advocacy, and collaborate with interdisciplinary teams to ensure each resident receives personalized, dignified care. You’ll be a vital part of our mission to promote emotional security, independence, and holistic well-being. What You’ll Do: Provide one-on-one counseling and emotional support to residents and families Guide residents through transitions, losses, and end-of-life issues with empathy Coordinate discharge planning and connect families with community resources Serve as Grievance Officer, resolving concerns with professionalism and care Complete MDS assessments and CMS-required documentation Facilitate care plan, transfer, and discharge meetings Collaborate with nursing, therapy, dining, and other departments to monitor resident needs Act as a liaison with hospice, home health, and mental health professionals Educate and support staff on geriatric social health and wellness Uphold resident rights, confidentiality, and HIPAA compliance Participate in continuing education and community-wide initiatives Requirements What We’re Looking For: Master’s degree in Social Work, Counseling, or related field Licensed Social Worker (or plan to become licensed within 6 months) Experience in retirement communities or CCRCs preferred Strong verbal and written communication skills Ability to build trust and show empathy in all circumstances Basic computer proficiency, including Microsoft Office Why Join Bishop Gadsden?
