To work under the direction and supervision of the Medicine Management Team (domiciliary care) in the provision of care to individuals in the community setting. To support the team with the provision of undertaking delegated tasks associated with Swanseas MAR scheme General administrative duties associated with the Swansea MAR scheme. Assisting the team in undertaking provision of warfarin MAR charts to service users in the domiciliary care setting. Taking blood samples from the service users and delivering timely to the laboratory for INR testing.
Communicate effectively with all patients, carers, families and members of the district nursing team and other significant agencies Assist and advise patients, carers and families to promote independence and self care.
