Description

A wonderful opportunity has arisen to join the award-winning Community Heart Failure service. The successful candidate must be able to assist in the management of heart failure patients as part of the integrated heart failure team across the patient pathway, which includes Patient home & clinic setting.
The role involves looking after a caseload of patients co-ordinating their care needs and optimising heart failure therapy through assessment, planning and implementing in conjunction with the Specialist HF team, liaising as required with the multi-professional team.
Share specialist heart failure knowledge and experience with clinical colleagues, provide patient and carer education & support to assist living with heart failure.
The successful candidate will also be part of the new heart failure annual review service, which aims to offer a specialist HF medication review, along with health promotion, lifestyle advice & HF education, with self-monitoring tools.
