JOB SUMMARY We are seeking an experienced Advanced Practice Provider (NP or PA) to join our Advanced Heart Failure team in a dedicated consult role. This position focuses on expert evaluation and management of patients with complex and decompensated heart failure and heart transplanted patients in the acute care setting. Additionally, the advanced HF APP will work closely with the HF transition team to reduce hospital admissions, readmissions, and LOS for patients in the ER and consulting services. EXPERIENCE AND EDUCATION Required Education Master's Degree Master's degree or higher in nursing, documentation of completion of accredited Advanced Practice Education Program. Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population. Licenses and Certifications (ANP) ADVANCED NURSE PRACTITIONER by Texas State Board of Nursing and Authorization for prescriptive authority by the Texas Board of Nursing. and Must hold and maintain current applicable life support certification(s) based on UT Southwestern guidelines for Advanced Practice Providers. Preferred Experience 1 year relevant clinical experience. JOB DUTIES Obtains and records complete medical history. Performs physical exams according to departmental protocols. Obtains and interprets laboratory tests according to departmental protocols. Assesses patient database and identifies biopsychosocial needs. Develops and implements patient management plans, following nursing processes and procedures identified in standing orders. Recognizes deviations from normal in-body systems and refers to appropriate medical/social resources. Initiates and manages preventative healthcare services. Provides relevant counseling and education to patients and significant others within role limitations. Facilitates teamwork through planned group processes, i.e. workshops, seminars, and staff meetings. Duties performed may include one or more of the following core functions: Directly interacting with or caring for patients Directly interacting with or caring for human-subjects research participants Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records) Regularly modifying, releasing or similarly affecting human-subjects research records. Performs other duties as assigned.
