We have an exciting opportunity for an enthusiastic, proactive person to join our Surgical CNS team as a Band 4 Cancer Navigator specifically to support our Hepatobiliary and Pancreatic Cancer patients through their treatment pathways.
The post holder will be required to work autonomously in managing patient initiated follow-up which includes maintaining the remote monitoring database. You will also need to be able to demonstrate excellent communication skills either over the telephone or face-to-face, where you will be responsible for carrying out Holistic Needs Assessments, sign-posting patients appropriately, escalation of complex issues to the Clinical Nurse Specialist Team as required and liaising with service users and multi-disciplinary teams,
The ideal candidate will be confident, self-motivated, be able to manage their time effectively, with the ability to display advanced IT, administrative and organisational skills. Competence in data collation and input is also required. Commitment and compassion for patients and the quality of care they receive will be imperative.

The Hepatobiliary and Pancreatic Cancer Navigator will form an integral part of the multidisciplinary team in caring for the physical, social, and psychological needs of patients and carers diagnosed with pancreatic, liver or gallbladder cancer, throughout their treatment pathway.
The post holder will have daily direct contact with patients and their families, providing support, advice, signposting, and assistance with the Living with and Beyond Cancer agenda. This will include the arranging and triaging of holistic needs assessments as part of their core role.
The post holder will be required to work autonomously in managing patient-initiated follow-up which includes maintaining the remote monitoring database, ensuring that all investigations are booked and performed as per guidance, results are escalated as required and MDT meetings are arranged and held as appropriate.
The post holder will work as part of the Hepatobiliary and Pancreatic multidisciplinary team under the guidance and supervision of a registered practitioner.
