Maternity Care Coalition (MCC) is seeking a Bilingual HFA Case Manager (Advocate IV) to deliver in-person, strength-based support to expectant parents and caregivers through the Healthy Families America (HFA) model.   What You’ll Do:  * Provide weekly in-home visits to families navigating challenges like trauma, housing insecurity, or child welfare involvement. * Use your language skills to ensure families understand their resources, rights, and parenting goals in a language that feels like home. * Support family well-being through evidence-based parenting and health education. * Create Family Service Plans, make referrals, and track progress through case notes and systems like ETO and SharePoint. * Represent MCC at community events, family meetings, and social groups that foster connection and support. Key Requirements:  * Associates, Bachelor's, or master's degree in social work, Public Health, or related field. * Bilingual fluency in English + Spanish or Haitian Creole (speak, read, and write). * Ability to work in-person and travel throughout Berks, Bucks, Delaware, Lehigh, Montgomery and Philadelphia counties. * Willingness to work non-traditional hours, including evenings and weekends as needed. * Valid U.S. driver’s license and clean driving record. * Comfortable working independently  Ideal Candidates Are:  * Culturally responsive. * Experienced in case management, early childhood education, or maternal/infant health. * Tech-savvy with the ability to document and manage family files digitally. * Ready to join an organization rooted in equity, belonging, and impact.  Perks & Benefits:  * Generous Paid Time Off – 3 weeks vacation + 1 week Winter Break + 12 weeks parental leave * Comprehensive Benefits Package – Medical, Dental, Vision, HRA up to $1500/year * 403B Retirement Plan with Employer Match * Wellness Programs & Pet Insurance * Tuition Repayment Eligibility – MCC is a Public Service Loan Forgiveness (PSLF) qualified employer * Ongoing professional development & DEI-focused training Why Join MCC?
