Job Details Job Location: Crestview Blaine - Blaine, MN 55434 Salary Range: $21.50 - $25.61 Hourly Job Shift: Day Shift The Home Health Aide – Home Care assists clients/residents with personal care, keeping in mind the goal of maintaining residents’ function at their highest level of independence. He or she observes residents daily for potential issues or changes in condition, and reports injuries/incidents to the supervisor immediately. RESPONSIBILITIES AND DUTIES: Communicates with coworkers and updates his or herself on resident needs, care plans, and status. Provides individualized, person-centered care that promotes resident functioning and independence at the highest level possible, including those with dementia/memory loss. Works alongside the nurse supervisor in preparing, reviewing, and updating assignments for each scheduled shift. Follows assigned instructions. Makes daily observations of change in client/resident condition and/or progress and documents any changes to the nursing supervisor and home health aide at change of shift. Responds to building-specific emergency call/pendant system and follows policy and procedures related to the calls, in a timely manner Assists clients/residents with medication reminders, administration, and other personal care duties as assigned, such as meal delivery, escorts, bathing, dressing, and grooming. Records pertinent information and documents on care performed in Point of Care, within PointClickCare EHR and EMAR system. Reports need for any repairs or cleaning to maintenance, housekeeping, or administration. Completes laundry (personal and linens) and distributes clean laundry, primarily during the overnight shift and as needed Serves residents/clients food in accordance with the Director of Culinary Services’ procedures and expectations for person-centered serving, including serving food in resident rooms and the dining room Assists the receptionist as directed by the supervisor Is able to work every other weekend and alternating holidays May complete additional duties such as mopping, cleaning, vacuuming, emptying trash, etc. Maintains knowledge of updated policies and procedures When care is not being provided, engages in life enrichment activities that are engaging and person-centered/individualized to each resident’s preferences. Maintains professional boundaries with residents, families, responsible parties, visitors, and any other person entering the Crest View community Documents involvement with critical residents and emergency situations, providing assistance if emergency situations arise. Participate in an ongoing Quality Assurance Program to monitor and evaluate the quality and appropriateness of resident services delivered, and identify any issues. Promotes teamwork among nursing staff and encourages acceptance and training of new employees. Maintains the safety of residents and employees Follows infection control guidelines when caring for residents, including those requiring precautionary measures or isolation. Completes timecard approval, and is present in accordance with attendance policy, and works as scheduled. Attends mandatory inservices, meetings, and annual required training. Acts professional in appearance and manner, wearing the official uniform and nametag at all times and has appropriate personal hygiene. Uses correct handwashing techniques and follows infection control processes. Has knowledge of and follows facility policies and procedures, Resident Bill of Rights, HIPAA, and Vulnerable Adult Act, observing confidentiality of residents, staff, and the facility. Other duties as assigned. Qualifications JOB QUALIFICATIONS: Ability to complete orientation and learn home care regulations, care techniques and competencies, infection control, and emergency procedures. Willingness in and experience working with older adults, including those with dementia/memory loss. Ability to work with people and establish satisfactory relationships with residents, families, providers, and coworkers. Ability to use time wisely, working with frequent interruptions and minimal supervision, while prioritizing and organizing duties. Ability to problem solve and be proactive. Excellent verbal and written communication skills, including electronic and telephone communication and ability to document resident care electronically or on paper. PHYSICAL QUALIFICATIONS: Must be able to communicate with staff, family, and residents and to use the telephone, fax, and electronic communication methods Must be able to document and maintain resident records, with the ability to read typewritten documents and observe the physical condition of residents. Must be able to perform lift 25 pounds or more, and must be able to move, assist, and/or help lift residents up to 400 pounds. Role requires frequent bending, squatting, stooping, sitting, walking, standing, pushing and pulling movements.
