Responsibilities Key Responsibilities / Essential Functions Receive, place and transfer calls using appropriate telephone etiquette; handles telephone information request with courtesy, accuracy, and respect for patient confidentiality. Ensure receipt of all Emergency Room, Clinical, Surgical Day Care, and Inpatient Observation records. Assemble content of record into established order. Analyze charts for proper physician documentation and signatures. Ensure that each form in the medical record bears the name and medical record number for that particular patient. Forward all records to coder. Assist with release of information to patient and/or families, representatives of insurance companies, or any other person who might present to the Health Information Department. Ensure that all records are checked in and out of the department. Ensure all lab, x-ray and/or other pertinent information received after discharge is scanned or filed on the appropriate record. Scan documents to the correct patient account number. Scan charts needed for coding audits. Monitor and edit documents in the transcription system. Assist in retrieving charts for physician completion. Pull records for other department personnel for chart review. Assist in filing patient records in the permanent files. Assist in purging of medical records. Ensure receipt of all birth certificate worksheets, enter information into the ADPH EBR, and secure parents’ signatures. Use policy and procedure manuals, and other reference materials, to ensure proper course of action in daily business operations. Complete all mandatory in-services annually. Demonstrate initiative toward being available to assist other hospital areas when the need arises. Perform additional duties as assigned.
