I understand that information regarding patients at this facility is confidential and is not to be disclosed without written authorization of the patient or authorized representative. I will discuss patient’s information only in the course of my duties and never in open areas such as hallways, snack bar, etc., where it might be overheard by visitors or other patients, or outside of the hospital while off duty. I also understand that information in some patient’s records may be governed by federal regulation and that disclosure of such information is punishable by fine or imprisonment, even after I leave the employment of the facility. I certify that I have completed HIPAA training and will honor all policies and procedures. I understand that violations of these policies and procedures may result in sanctions. Position Summary: This position is responsible for the daily processing of medical records which includes: scanning paper medical records in the EMR, preparing scanned records for storage, assist in maintaining accurate MPI, validate interfaced reports into EMR, verify successful transmission of auto-faxed reports, assist with release of information; answers and assists with phone calls, and other duties as assigned. II. Position Requirements:   1. Licensure/Certification/Registration: N/A   2. Education: Minimum High School Diploma or GED. Medical terminology preferred.   100. Experience: Incumbent should have previous clerical experience; preferred in a hospital or medical office setting. Proficient computer/keyboarding skills, faxing, filing, scanning, copying, and telephone etiquette. Three months on-the-job training will be provided to orient the individual to hospital policy and procedures.   500. Special qualifications: Incumbent must possess exceptional organizational skills with a strong attention to detail and demonstrate the ability to follow guidelines. Must be able to manage the quality, accuracy and timeliness of documents processed. Must maintain strict confidentiality of all patient medical records and abide by HIPAA Regulations. Must be dependable with strong work ethic. Must be a quick learner and able to work independently; as well as interact with physicians, patient care team members, and the public in a courteous and professional manner. Proficient in the computer program Microsoft Office (Excel, Word, Outlook). Able to manage and prioritize multiple tasks simultaneously. Must be able to sit for long periods of time while remaining focused and organized. Incumbent must possess ability to work under pressure with time constraints.   III. Degree of Supervision Required: Minimum; incumbent must be able to work very well on their own. IV. Ages of Patients Served: The staff member must be able to demonstrate the knowledge and skills necessary to provide care, based on physical, psycho/social, educational, safety and related criteria, appropriate to the age of the patient served in his/her assigned service area. The skills and knowledge needed to provide such care may be gained through education, training or experience.
