We are looking for a Registered Nurse (RN) and a CDM (Chronic Disease Management) Nurse Registered Nurse for Myhealth Medical Centre in Blacktown - two positions
Myhealth Blacktown is bulk billing medical centre, conveniently located inside shopping centre . We are a busy popular medical centres and need someone who is available for immediate start.
We are currently recruiting for an experienced and passionate Registered Nurse and a CDM (Chronic Disease Management) Nurse Registered Nurse for Myhealth Medical Centre in Blacktown.

  • Experience and qualifications to complete Pap Smears
  • Knowledge of Sterilisation Procedures

  • Cold Chain Monitoring
  • Data management and documentation

ROLE & RESPONSIBILITIES - CDM (CHRONIC DISEASE MANAGEMENT) NURSE

  • Patient-centred care & coordination

CDM Nurses work closely with patients who have one or more chronic health conditions, especially those recently hospitalized or at risk of hospital admission. They help individuals stay well at home by coordinating care and connecting them with needed services nnswlhd.health.nsw.gov.au+1.

  • Collaborative care planning

These nurses assist in creating Shared Care Plans in collaboration with GPs and other healthcare providers. This helps ensure ongoing, tailored care in the community nnswlhd.health.nsw.gov.au+1.

  • Developing care plans & assessments

In primary care settings—such as general practices—they prepare Care Plans and Team Care Arrangements, perform health assessments, and support behaviour change to help patients manage chronic conditions effectively.

  • Patient empowerment & review

CDM Nurses empower patients to take ownership of their health, conduct regular reviews of care plans, and ensure continuity and relevance of care
