Responsibilities

YOUR NEW ROLE:

As our HITH Clinical Coordinator, you will be the clinical backbone of our Hospital in the Home service. You will blend your clinical expertise with operational leadership to ensure the delivery of safe, efficient, and profoundly impactful patient care. This is a pivotal role where your clinical judgment guides our team, your operational skills ensure seamless daily operations, and your leadership helps grow a vital service.

WHAT YOU’LL DO:

  • Provide Clinical Leadership: Oversee and guide the clinical care provided by our HITH nursing team ensuring it meets the highest.
  • Solve Complex Challenges: Manage clinical risk, coordinate critical medication supplies, and respond to incidents to ensure unwavering patient safety.
  • Drive Operational Excellence: Create and publish efficient staff rosters, manage resources wisely, and contribute to systems that make our service run smoothly.
  • Be a Key Connector: Build strong, trusted relationships with referring hospital teams, specialists, patients, and families, acting as the professional face of our HITH service.
  • Measure & Improve: Track key performance indicators like patient satisfaction and readmission rates, using data to continuously enhance our care and impact.
