Your Role & Impact The HIV/AIDS Case Manager facilitates a collaborative process to provide individualized case management services to HIV-positive clients that includes intake, assessment of client needs, care planning and psychosocial referrals, advocacy, partner services, HIV testing, and health education. What We’re Looking For Bilingual in English and Spanish. Bachelor’s degree in Social Work, Public Health, or Social Sciences or three (3) years of work experience in a directly related field. At least one (1) year recent experience in a healthcare setting. Knowledge and experience with public/private benefits and entitlement programs. Experience with brief interventions and case management techniques. Experience working with low income, diverse populations. Patient advocate; facilitator; negotiator skills. Computer proficiency with Microsoft Office suite products (Outlook, Word, Excel, and PowerPoint) and electronic health record systems. Knowledge of issues and community resources related to HIV/AIDS, chemical dependency, mental illness, housing, health insurance, and other social services. The pay range for this role is $25.30 - $35.42 on an hourly basis based on experience and internal equity.
