SUMMARY

Provides individualized or group education and support (not mental health treatment) to clients, families, and/or friends including education on medical issues, medication issues, and benefits eligibility.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
1.Assist eligible clients that are newly diagnosis or have fallen out of care, with mental and emotional support, relevant education, adherence to mediations, and navigation of the medical care system.
2.Research and remain up-to-date on recourses available to the client’s specific needs and provide linkages between clients and third-party providers.
3.Identify and report to management client feedback on barriers to care, areas requiring improvement, and service recommendations.
4.Link clients to appropriate support services.
5.Research and acquire up-to-date knowledge in living with HIV and treatment and prevention developments.
6.Work closely with case managers, referral specialist, linkage coordinator, and clinical staff to provide up-to-date information such as lab work reports and emerging issues that clients might be facing.
7.Connect and relate to the clients by disclosing personal HIV status, relating to the individual client’s journey with HIV, offering support and advice on individual client’s daily concerns.
8. Accompany clients at medical appointments and related ancillary services, as needed, for:
a.Maintaining support, record of changes on the client’s health b.Discuss best health care choices with clients and their doctors.
9.Encourage clients to make strength-based choices to improve health outcomes.
10.Organize and co-facilitate bi-weekly support group, in accordance with contracted deliverables and agency identified goals.
11.Perform other Prevention Department duties including by not limited to: HIV testing and counseling, STI/STD testing and counseling, linkage to care, referral to PrEP services, outreach and community education, condom distribution, and behavioral intervention facilitation.
12.Promote and link eligible clients to agency’s medical clinic and patient assistance program.
13.Work and collaborate with Development Department to increase promotional and marketing messages, as well as maintain up-to-date, relative, and culturally competent promotional materials.
