Description Conduct HIV pre/post test counseling sessions. Member of the community outreach team via mobile testing van. ? Perform HIV risk assessments. ? Provide health education and risk reduction counseling. Conduct Screening, Brief Intervention, Referral and Treatment (SBIRT) services in the community. ? Collect information relevant to the contract services necessary to identify client needs, assess the clients’ primary care status and develop service plans. Provide re-engagement efforts, face to face or via phone, mail, etc. to reconnect clients who have not maintained regular contact with the program. Ensure connections made for clients to link them with HIV primary care, and for needs such as social and case management services, mental health counseling, housing services and access to benefits. ? Provide one to one session to orient individuals to the HIV service system, including the all-agency programs and the benefits of engaging in continuous care. Conduct control procedure for Rapid HIV Testing. Perform unscheduled interventions or other appropriate assistance with unexpected occurrences or circumstances that exceed the client’s normal range of coping strategies. Maintain good working relationships with all staff, clients and visitors. Attend staff development training. ? Attend meetings as assigned. Attend annual in-service as required or mandated. ? All other duties as assigned by the Director or designee. Requirements High school diploma/GED with at least one (1) year of experience working in the field of HIV/AIDS. CASAC training, a plus Certification in HIV Pre/Post-Test Counseling, a plus. Knowledge about HIV/AIDS, STDs and the Risk Reduction Model. Computer literate and experience working with electronic medical records. Excellent written and oral communication skills. Ability to work flexible work schedule (including evenings and nights) Ability to interact effectively with all populations served. Ability to speak read and write the English language. Bilingual English/Spanish, a plus.
