As part of a mission-driven organization, you will play a key role in empowering individuals to reclaim their lives and build healthier living environments.

In this role, you will:

  • Collaborate with Home Coaches and Therapists to develop individualized care plans for clients.
  • Conduct client inquiries and in-home assessments to understand clients’ physical, social, and financial needs.
  • Assist clients in accessing essential services, such as Medicare, food assistance, family care, and disability programs.
  • Monitor client progress by evaluating treatment outcomes, advocating for additional resources, and providing personal support during crises.
  • Maintain accurate records by reviewing case notes, logging events, and documenting reports.
  • Ensure quality service delivery by partnering with program staff and external service providers
