APH offers customized services for Medicaid, Medicare, and the ACA/Marketplace lines of business.       POSITION PURPOSE     The Home- and Community-Based Operations Manager is a key leadership role responsible for the successful implementation, management, and optimization of home- and community-based respiratory care programs. This position oversees complex operational functions, program development, quality improvement initiatives, and client relationship management to achieve measurable health outcomes for diverse patient populations.  This role requires expertise in Population Outcome Management, Complex Respiratory Management, Enhanced Respiratory Care, Quality Enhancement Initiatives, and Remote Monitoring, with a deep understanding of how these components integrate into post-acute and home healthcare delivery models.  The Operations Manager will collaborate with senior leadership, clinical teams, and external partners, including managed care organizations (MCOs), medical directors, durable medical equipment (DME) providers, and community-based agencies—to ensure that every program meets its clinical, operational, and financial objectives. This position also ensures alignment with evidence-based best practices, regulatory compliance, and patient-centered care principles, while leading teams toward continuous performance improvement.        DUTIES & RESPONSIBILITIES    Program Development & Implementation  * Lead the design, launch, and execution of home- and community-based respiratory care programs that align with organizational goals and client requirements. * Partner with customers, senior leadership, and interdisciplinary teams to define programobjectives, timelines, and success metrics. * Develop and implement standardized workflows, documentation tools, and reporting processes to ensure program efficiency and compliance. Clinical Oversight & Quality Management  * Provide clinical oversight for Home and Community-Based Programs, including Complex Respiratory Management, Katie Beckett Program, and Individual Assessment Navigation. * Ensure timely and accurate completion of monthly member assessments (via chart review or in-person visits) to support care planning and program objectives. * Monitor, analyze, and report on key performance indicators (KPIs), including patient outcomes, hospital readmission rates, and adherence to care plans. * Identify and implement evidence-based practices to improve patient outcomes, operational efficiency, and cost-effectiveness. * Oversee quality improvement initiatives and track the impact of interventions on patient care and resource utilization. Stakeholder Engagement & Customer Relationship Management  * Serve as the primary point of contact for high-priority client interactions, including medical directors, quality directors, and nurse managers. * Facilitate regular communication with stakeholders to share program outcomes, identify areas for improvement, and address member-specific concerns. * Collaborate with home equipment providers to ensure timely delivery and proper utilization of respiratory devices and related technology. Team Leadership & Development  * Provide daily operational oversight and guidance to the Respiratory Care Coordinator team, ensuring they have the tools and resources to meet program goals. * Conduct weekly clinical team meetings to review successes, address challenges, and share best practices. * Foster a culture of accountability, collaboration, and continuous learning through consistent feedback, training, and mentorship. Operational Excellence & Reporting  * Develop and maintain detailed tracking systems for member progress, program performance, and resource allocation. * Deliver comprehensive monthly productivity reports and KPI analyses to senior leadership and customers. * Evaluate program processes for efficiency, scalability, and financial sustainability; recommend and implement cost-saving measures. * Ensure adherence to all regulatory, ethical, and confidentiality standards related to patient care and data management.     SKILLS & QUALIFICATIONS  * Five (5+) years of experience in respiratory care. * Three (3+) years in a leadership or program management role strongly preferred. * Proven ability to manage multiple high-priority projects simultaneously in a fast-paced healthcare environment. * Strong clinical knowledge of respiratory care in post-acute and home-based settings, including LTACH, SNF, and home care. * Expertise in home medical equipment related to respiratory management and the ability to assess appropriateness of prescribed equipment. * Understanding of Social Determinants of Health (SDOH)and their impact on respiratory care program success. * Skilled in developing data-driven solutions to improve patient outcomes and program efficiency. * Exceptional communication, interpersonal, and relationship-building skills, with the ability to collaborate across clinical and administrative teams. * Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and capable of working with electronic health records (EHR) and remote monitoring technologies. * Strong organizational skills with the ability to meet deadlines, prioritize tasks, and adapt to changing demands. * Knowledge of regulatory frameworks and HIPAA-compliant data handling practices. * Reliable transportation for patient visits.   EDUCATION, LICENSES, & CERTIFICATION  * Associate’s degree in healthcare administration, respiratory therapy, or related field required. * Bachelor’s degree in healthcare administration, respiratory therapy, or related field preferred.. * Graduate of an approved American Medical Association (AMA)-accredited school for respiratory care by the Commission on Accreditation for Respiratory Care (CoARC). * Current, active, and unencumbered state licensure as a certified respiratory therapist, registered respiratory therapist, or respiratory care practitioner (CRT/RRT/RCP)from the National Board for Respiratory Care (NBRC).  * Valid and unrestricted driver’s license. * Proof of current auto insurance coverage.   BENEFITS & TOTAL REWARDS  * Paid Orientation and Training  * Insurance – Medical, Dental, Vision, and Life * 401k Plan – 3% match * Employee Assistance Program * Tuition Reimbursement * Continued Education Support * Mileage Reimbursement (if applicable) * Referral Bonuses * Paid Holidays (9days) * Paid Time Off(20 days) * Paid Volunteer Hours      CHARACTER & COMPETENCIES  * Courage– To have the courage to the right thing at the right time. * Ownership– To take ownership of every issue you touch. * Respect– To respect yourself, co-workers, and for those whom you care. * Excellence– To be excellent in all that you do. * Diversity - Demonstrates knowledge of EEO policy;
