The Home-Based Primary Care Director is responsible for the strategic, operational, and financial performance of Unity’s Home-Based Primary Care (HBPC) program. This role ensures that interdisciplinary team members provide high-quality, efficient care to patients while maintaining compliance with state, federal, and payer regulations. The Director partners closely with organizational leaders to align financial stewardship, workforce stability, regulatory compliance, and patient care excellence. Schedule: Monday–Friday, 1st shift, full-time salaried position scheduled within the hours of 7:00 a.m. – 5:00 p.m. Some evening and weekend commitments may be required. This position offers a hybrid work arrangement, with the primary in-office location at Unity’s Administration Building in De Pere. Knowledge, Skills, and Abilities Knowledge of Medicare and Medicaid billing and reimbursement models Experience with Electronic Health Records (EHR) such as WellSky or Epic Proficiency in Microsoft Office (Word, Excel, Outlook) and scheduling systems Experience collaborating with cross-functional healthcare teams Familiarity with workforce performance management and labor analytics Ability to interpret financial data, support budget planning, and drive operational improvements Strong leadership, communication, organizational, and critical thinking skills Adaptability to changing priorities while maintaining attention to detail Commitment to staying current with healthcare policies, regulations, and evidence-based practices Compliance with all applicable laws and Unity policies, including required training What You’ll Do: Strategic Planning & Program Development: Lead HBPC strategic initiatives and program growth, including rural expansion and integration with health systems, ACOs, and payers Build and maintain partnerships with hospitals, skilled nursing facilities, community organizations, and payers Track and report key performance indicators (KPIs) such as patient census, referrals, and cost-effectiveness Program Growth & Innovation: Use data and technology to improve operational efficiency and program scalability Integrate telehealth, remote patient monitoring, and analytics into workflows Shift care models to proactive, team-based approaches for high-risk patients Develop financial models to manage expenses, reimbursement, and revenue Operational Management: Oversee daily operations including staff scheduling, caseloads, and workflows Manage budgets, monitor P&L, optimize reimbursement, and negotiate contracts Analyze labor and productivity data; align staffing with patient census and workload Develop dashboards and reports to support data-driven decisions Team Leadership & Supervision: Recruit, train, and supervise interdisciplinary team members (RNs, NPs, MAs, Social Workers) Lead, coach, and provide feedback through structured meetings and performance reviews Participate in hiring and performance management processes Conduct annual EMPOWER meetings and ensure payroll accuracy Quality Improvement: Monitor KPIs such as patient satisfaction, readmission rates, and productivity Address patient grievances and ensure high-touch, relationship-based care. Partner with Quality to ensure compliance with CMS, state, and regulatory standards Identify operational gaps and lead improvement initiatives Why Unity? Requirements Bachelor’s degree in Healthcare Administration, Business Administration, Nursing (RN), or Social Work required; Master’s degree preferred. Minimum three (3) years of progressive leadership experience in home health, hospice, ambulatory care, or healthcare operations. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
