Description
UPMC Home Healthcare and Family Hospice is hiring a Home Care Aide to support Erie County. This is a Monday through Friday, 8:00 AM-4:30 PM position with rotating weekend shifts, providing exceptional care to patients in their place of residence. This is a field position, making home visits to both home health and hospice patients. Responsibilities: Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff. Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals. Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. Proven ability to facilitate change. Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. Participates in all required agency meetings. Must maintain annual competency evaluation scores as determined by regulatory requirements Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. High School Diploma or GED - Required One (1) year of experience - Preferred Certified Nurse Aide - Preferred Minimum of 75 hours of Nurse Aide training - Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
