UPMC Western Maryland is seeking a Casual (As-Needed) Home Care Aide (CNA) to support our Home Health located in Cumberland, Maryland! This is a dayshift position, no weekends or holidays required! As the Home Care Aide (CNA) you will provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. You will also function as an extension of skilled nursing services and/or rehabilitation services in the home. You will cover Allegany County Maryland. Responsibilities: Attends compliance training and adheres to the organization standards of conduct, policies and procedures. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional. Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization. Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. Proven ability to facilitate change. Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. Participates in all required agency meetings. Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional. Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff. Participates in all required agency meetings. Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization. Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
