UPMC Family Hospice is hiring a Home Care Aide for a casual position to cover Lycoming, Union, Northumberland, Snyder, Sullivan, Clinton, Columbia, parts of Centre, Montour counties 16 hours every pay period. -rotating Holidays and weekends Weekend rotation every 4th weekend Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home. Responsibilities Must maintain annual competency evaluation scores as determined by regulatory requirements Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. Proven ability to facilitate change. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. Participates in all required agency meetings. Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient’s condition to appropriate professional staff. Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional. High School Diploma or GED - Required One (1) year experience - Preferred Certified Nurse Aide – Preferred Minimum of 75 hours of Nurse Aide training – Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
