THE ROLE Within a Primary Health Care Framework, and in accordance with Agency policies and legal requirements, works as a member of the multidisciplinary team to coordinate the provision of support services to enhance the quality of life for the frail, aged and people with disabilities living on the West Coast. DETAILS OF APPOINTMENT Fixed term full time, day worker position, working 76 hours per fortnight, commencing from as soon as possible for 3 months.
