Lutheran Services is seeking a dedicated and client-focused Home Care Coordinator to join our team.

This full-time role supports clients across three Home Care sites – Trinder Park (Woodridge) St Andrews (Tallebudgera), & Zion (Nundah) helping older people remain independent and connected to their community.

  • Location: Trinder Park, Woodridge (with travel to Nundah & Tallebudgera) QLD. Pool vehicle will be available for client visits and travelling to other sites.
  • Employment Type: Full Time
  • Remuneration: $38.18 – $38.74 per hour + Super + Salary Packaging+ Employee Discounts & Benefits

ABOUT YOU:

We’re looking for someone who brings:

  • Experience in Home and Community Care, Aged Care, or Case Management
  • A strong understanding of aged care compliance and quality service delivery
  • Excellent communication, planning and coordination skills
  • A client-first mindset and commitment to supporting older people to live independently
  • A current QLD Driver’s Licence (essential)
  • A Police Check and NDIS Worker Screening (or willingness to obtain)

ABOUT THE ROLE:

This is a rewarding opportunity for a professional who is passionate about aged care and thrives in a role that blends client engagement, planning, and operational coordination.

As a Home Care Coordinator, you’ll play a key role in:

  • Supporting client independence through quality care planning, goal setting and budget management
  • Coordinating and delivering services that align with aged care standards and individual client needs
  • Maintaining positive client relationships and liaising with carers, family, and external service providers
  • Ensuring compliance, continuous improvement, and service excellence across sites
  • Collaborating with team members to deliver safe, person-centred home care
  • Participating in onboarding, offboarding, and ongoing assessment processes
  • Supporting client visits across all locations (a pool vehicle is available for travel)
