TERM (ONE YEAR) FULL-TIME POSITION IN SQUAMISH VALLEY CLOSES: Open Until Filled BE PART OF SOMETHING MEANINGFUL Are you a compassionate and skilled nursing professional with a passion for supporting holistic, community-based care? Squamish Nation is seeking a Home Care Nurse Assessor to join the Yúustway Health and Wellness Department within the Nexwnínlhewá7nem Ch'áwch’aw People Services Division. This role plays a vital part in supporting the health, independence, and wellbeing of community members by coordinating and assessing home care services. More than a clinical role, this is an opportunity to deliver culturally safe care and build meaningful relationships with clients, families, and community partners. WHAT YOU’LL DO * Develop and implement client-centered care plans with input from families and community supports. * Coordinate home care services in collaboration with the Home and Community Care team and allied health professionals. * Conduct client assessments and maintain accurate, confidential health records. * Provide clinical guidance and supervision to Health Care Assistants and Homemakers. * Build and maintain strong relationships with internal teams, external providers, and community partners. * Advocate for clients and support access to appropriate health services and referrals. * Participate in program planning, development, and evaluation activities. * Promote culturally safe, respectful, and holistic care practices. * Participate on committees and contribute to continuous improvement initiatives. * Perform additional duties as required to support departmental goals. WHAT WE’RE LOOKING FOR * Bachelor of Science in Nursing (BSN) preferred; LPN with relevant experience may be considered. * Current registration and good standing with the BC College of Registered Nurses. * Minimum 3 years of relevant nursing experience in community or public health settings. * Strong understanding of client assessment, care planning, and case coordination. * Knowledge of EMR systems and intermediate proficiency with Microsoft Office. * Indigenous Cultural Safety training (or willingness to complete). * Excellent interpersonal, communication, and relationship-building skills. * Valid First Aid and CPR certification (or willingness to obtain). * Valid BC Driver’s License and access to a reliable vehicle. * Ability to travel to Squamish Valley and work flexible hours as required. * Successful completion of a Criminal Record Check (Vulnerable Sector). * Experience working in a First Nations community is an asset.
