Responsibilities

  • Collaborates with the health care team on all aspects of service delivery;
  • Receives and sends referrals, conducts nursing assessments and creates care plans to meet client needs and follow treatment orders;
  • Works with the health team to develop a client work plan where required that promotes the client and family ability to function at the highest level of independence possible;
  • Completes referrals for clients requiring additional services;
  • Advocacy role for clients to community and institution-based services;
  • Performs nursing tasks as required in the client’s home that are in line with the Home and Community Care Program
  • Performs nursing assessments on new and continuing clients (at minimum yearly);
  • Coordinates case management with client, family, and other service providers;
  • Provides interdisciplinary, needs-based palliative care to clients
  • Direct supervisor for the Home Support Workers which includes new hire orientation, work tasks distribution, performance and time management.
  • Facilitates the Nunatsiavut Home Support Worker training program with Home Support Workers as needed;
  • Carries out Diabetes care, including foot care, and follow up according to the HCCP policy;
  • Maintains linkages with Community Clinics, Labrador Grenfell Health (LGH) and other client service providers as required;
  • Promotes self-care and independence of clients through a population health approach and the adoption of Primary Health Care principles;
  • Maintains records on clients, and prepares and submits monthly reports;
  • Participates in research and evaluation as requested;
  • Acts as a preceptor and actively participates in the coordination of education programs for nursing students as required;
  • Participates in local, regional, or national committees as required to promote excellence in client care and professional development;
  • Coordinates to meet client’s needs through the ordering of supplies and equipment in concert with the NIHB program
  • Participates in professional development to maintain high professional standards;
  • Is involved in the development and maintenance of nursing policy as requested;
  • Provides coverage for the Public Health Nursing program as required;
  • Works flexible hours where required;
  • Other related duties as required;
  • Travel as required.

Qualifications

  • Bachelor of Nursing Degree and eligibility for Registration with the College of Registered Nurses of Newfoundland and Labrador with a minimum 2 years of related experience, including Acute Care, Diabetes Care and Community Health Nursing.
  • Current CPR certification or an ability to be trained;
  • Must possess strong interpersonal skills;
  • Must be self-directed, able to work with minimal supervision, and possess excellent written and oral communication skills;
  • Requires a good working knowledge of computer programs such as Word, Excel, PowerPoint, etc;
  • Must be able to demonstrate a client-centered approach, with proven teaching and facilitation skills.
