Job Details Job Location: Cullman Regional Medical Center - Cullman, AL 35056 Position Type: Full Time Job Shift: Any Job Summary: Review home care and hospice SN, AIDE, MSW daily notes for accuracy as needed. Analyze data to present quarterly QAPI data for home care and hospice. Maintain up-to-date QAPI data to lead QAPI meetings with leadership. Maintain Emergency preparedness for home health and hospice employees/patients. Provide feedback to home care and hospice supervisors for needed QA improvements. Attend education classes, IDT, and case conference as needed. Assist hospice and home care teams as needed in day-to-day operations. Provide one-on-one education/QA review with appropriate clinicians. Demonstrate and encourage team behavior and exceptional patient/guest experiences. Uphold and promote patient safety and quality. Qualifications Education: License in practical nursing or associate degree in nursing required. Experience: Minimal 2 years’ experience as home care field nurse required. Preferred experience in hospice. Preferred hospital med/surg; ER or critical care experience. Must have extensive knowledge in Home Health Oasis and regulatory guidelines for all quality measures. Additional Skills/Abilities: Home Health oasis data knowledge is a must; Leadership skills; great communication skills; critical thinking skills; knowledge in use of Microsoft and Excel; flexibility and ability to multitask and prioritize. Must be organized and driven to complete tasks by a deadline.
Responsibilities
The Home Care Quality Coordinator/Educator is responsible for reviewing daily notes for accuracy, analyzing data for quarterly QAPI presentations, and maintaining up-to-date QAPI data. They also provide feedback for quality improvements and assist teams in day-to-day operations.
