Job Details Job Location: Sonoma Building - Eureka, CA Position Type: Full Time Salary Range: $21.00 - $21.00 Hourly Job Shift: Day Description Humboldt Senior Resource Center is looking for a Home Care Scheduler to support our Home Care services in our Redwood Coast PACE program. The schedule for this position is Monday through Friday, 8 am to 5 pm. JOB SUMMARY: The Home Care Scheduler (Administrative Assistant III) is responsible for coordinating and maintaining the daily schedule of in-home services for PACE participants. Working under the supervision of the Home Care Manager, this position ensures that authorized care hours are accurately assigned, CarePartners and contracted providers are scheduled efficiently, and participant needs are met in accordance with their individualized Plans of Care. The Scheduler serves as a key communication hub between participants, families, CarePartners, contractors, and staff, supporting service delivery, compliance, and continuity of care. ESSENTIAL DUTIES AND RESPONSIBILITIES: Maintains live scheduling calendar for Home Care services. Maintains master list of all participants and the current authorized in-home care hours. Schedules/reschedules in-home CarePartner assignments. Receives and responds to telephone calls from Home Care vendors and CarePartners; directs to Home Care Manager as needed. Assists Center Manager and Home Care Manager in coordinating and scheduling contractor and employee trainings. Assists Home Care team in monitoring Home Care hours and duties as provided by the participant’s Plan of Care. Reports significant participant condition changes to Home Care Manager. Follows agency policies for incident reports, grievances and appeals. Communicates effectively with participants, family members, caregivers, staff, and visitors. Completes documentation accurately and in a timely manner. Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence. Effectively collaborates with staff, peers, and contractors to meet PACE goals and further success. Participates in on-call coverage in conjunction with the Home Care team. Demonstrates behaviors consistent with agency values: Respect, Patient, Courteous, Tolerant, Friendly, and Teamwork.. Additional duties may be assigned, as needed, that align with the responsibilities of the position and support the vision, mission, and values of the organization. Qualifications EDUCATION &/or EXPERIENCE: High school diploma or GED required. One (1) year working with a frail or elderly population preferred. PRE-SCREENING REQUIREMENTS: This position requires the successful completion of a background check. Pre-employment physical, and PPD skin test (TB screening). COVID-19 and Influenza (flu) vaccines preferred. JOB SKILLS: Customer service experience, skills, and abilities necessary to work and communicate with diverse frail and/or elderly populations, employees, visitors, the general public, with the ability to assist people who are in crisis or under extreme stress. Strong verbal and written communication abilities; able to convey information clearly and professionally to staff, participants, families, and external partners. Basic understanding of California wage and hour laws related to rest and meal breaks, overtime, and time pay. Builds positive relationships with diverse populations, including aging and dependent adults, families, caregivers, staff, and community stakeholders. Works effectively as part of a multidisciplinary team demonstrating sound listening skills to receive directions and understand participant concerns; maintains a cooperative and supportive attitude. Applies good judgement, resourcefulness, and flexibility to resolve issues and adapt to changing circumstances. Maintains confidentiality, reliability, and ethical standards; consistently demonstrates punctuality, attendance, and responsibility in work performance. Maintains participant privacy and organizational confidentiality, adhering to HIPAA and agency standards. Remains calm and effective when priorities shift, deadlines change, and unexpected issues arise. Ability to collect data, identify pertinent information, and prepare reports and presentations. Highly organized and able to multi-task. Ability to work independently and with minimal supervision. Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and relevant computer programs and software (e.g. electronic health record software). Basic knowledge of web-based applications such as timekeeping and training programs. PHYSICAL REQUIREMENTS: The Home Care Scheduler primarily works in an office environment with frequent use of a computer, telephone, and other standard office equipment. The position requires the ability to sit for extended periods, perform repetitive hand and wrist motions, and maintain focus while managing multiple tasks. Occasional standing, walking, bending, or reaching may be necessary to complete office-related duties. The role also involves frequent communication - both in person and over the phone - requiring clear verbal and written skills. Light lifting of up to 20 pounds may be required on occasion.
