QUALIFICATIONS:

✔ Associate’s degree in human services or related field (experience may substitute)
✔ 1–2 years of related experience; experience with elderly or disabled populations preferred
✔ Strong communication and organizational skills
✔ Valid driver’s license, reliable transportation, and minimum liability insurance coverage of $50,000/$100,000
✔ Must pass a background check and have no history of abuse, neglect, or exploitation

ABOUT THE ROLE:

As the Home Care Services Coordinator, you’ll be the vital link between clients and caregivers. You’ll coordinate services, manage schedules, handle referrals and intakes, support care planning, and ensure everything runs smoothly — all while delivering compassionate, client-focused service.

WHAT YOU’LL DO:

Serve as the first point of contact for clients and direct care staff
Schedule and coordinate home care services
Support intake, assessment, and care planning processes
Ensure documentation and EVV compliance
Participate in direct service observations and quality assurance
