JOB SUMMARY

Under administrative direction, the incumbent in this position is responsible for clinical consultation, development, coordination, oversight and evaluation of Medical Assistance home care and self-directed services. Due to the complexity of clinical decision-making, preference is given to education and experience as a Public Health Nurse (PHN), and a Registered Nurse (RN) is required to ensure high standards of care and compliance with clinical best practices.

Responsibilities include:

  • Lead and participate in division, administration-wide and department-wide strategic planning, leveraging clinical expertise to ensure that strategic goals align with evidence-based nursing practices, patient care standards and compliance with federal and state laws
  • Establishing goals for long-range planning projects, incorporating clinical insights to improve patient outcomes and service delivery efficiency
  • Defining project scope, expected benefits, goals and deliverables that support division priorities in collaboration with DSD and administration management, internal and external partners
  • Coordinate administration-wide policies related to programs and projects
  • Developing, executing and revising, as appropriate, project charters and work plans
  • Providing lead work for Home Care Services and other team work and projects
  • Consult and advise related to Minnesota’s Nurse Practice Act including regulations related to delegation to unlicensed personnel, allowances and limitations or LPN and RN practice
  • Lead all legislative work for the team including developing language and TA, working on division/department proposals, collaborating with legislative staff, responding to legislative and fiscal inquiries, preparing bill analysis documents including potential outcomes, adherence to federal and MN home care laws, costs and equity reviews.
  • Taking a leadership role in working across the division and the department as well as with external partners to ensure a transparent process in the design and implementation of projects, including implications for nursing practice and upholding the MN Nurse Practice Act
  • Communicating to team members and partners including development of written progress reports and oral presentations
  • Leading work with federal relations and other areas of DHS for CMS state plan amendments and amendments to other authorities (including waivers) for changes that impact home care and self-directed services. Provide consultation to federal relations and other areas of DHS related to interpretation of nursing practice and how to implement federal changes within MN nursing practice and home health care regulations

Duties include administration of complex and intersecting programs, identifying, analyzing and resolving service or quality issues, and documenting requirements, policies and procedures for internal and external communications including web-based manuals. The incumbent will also provide training, technical assistance, consultation to other areas of DHS, other state agencies, service providers and lead agency personnel.

MINIMUM QUALIFICATIONS

Current licensure as a registered nurse (RN) AND at least 4 years of clinical experience developing and overseeing home care services. Experience must demonstrate the following:

  • Medical Assistance and long-term services and supports in Minnesota including clinical expertise in interactions with nursing practice and home care agency requirements
  • Project management principles and practices to determine program direction, plan work, set goals, monitor, and evaluate accomplishments
  • Understanding and implementing complex healthcare regulations

PREFERRED QUALIFICATIONS

  • Informed clinical decision-making and ability to provide expert consultation
  • Advanced knowledge of MMIS (Medicaid Management Information System)
  • Demonstrate advanced skills in Microsoft Office Suite (Word, Excel, Access, PowerPoint, Visio, SharePoint), remote communication, and web communication technology
  • Variety of experiences working effectively with others from different backgrounds and/or cultures
