We are looking for a Home Care Specialist, Integrated Care Solutions who can be autonomous in leading a team of Personal Support Workers / Nurses/ CTC’s, and comfortable providing support to staff and clients. Our next Home Care Specialist will thrive in a forward-thinking culture where being a self-starter and taking initiative will drive the success of their role leading the team.
This is a full-time permanent role, on-site based in our East Toronto office in conjunction with traveling for client visits.

SKILLS NEEDED FOR SUCCESS:

  • Undergraduate degree in Nursing from a recognized accredited school of nursing
  • Active registration and in good standing with the College of Nurses of Ontario, as a Registered Nurse(RN).
  • 2-3 years of clinical nursing experience, preferably in a supervisory or leadership role.
  • You are a highly energetic and enthusiastic individual who loves to work in a dynamic and fast paced environment.
  • Knowledge of home care delivery, gerontology, and integrated care models.
  • Highly organized, with ability to multitask conflicting priorities.
  • Demonstrated leadership, clinical, and communication skills.
  • Experience working in a home/community-based setting is a nice to have.

Responsibilities
  • You will be overseeing service provider personnel and ensuring compliance with policies on client safety, risk reduction, and staff safety.
  • You are driven and thrive off delivering the best service possible to staff and clients.
  • You will assess, plan, collaborate with the client, family, and other members of the health care team to establish and/or update the care plan and implement and evaluate client care with every client encounter and provide clinical advocacy
  • You will use a variety of nursing skills such as IV antibiotic administration, IV Cannulation, Chemo disconnects, CVAD care, and PORT-A-CATH care access/de-access, Wound Care, Intravenous Care, Diabetes Management Support, CVAD, and Drain Care management to name a few.
  • You will be responsible for the day-day management and coordination of the services your staff will provide to the community.
  • You thrive on being a self-starter and can work well under minimal supervision.
  • You can work in a team but are proven to be reliable while working independently.
  • You are a problem solver at heart and when problems do arise you are able to find solutions and able to provide the best customer service possible.
