We are looking for an experienced and compassionate Co-Care Partner to coordinate high-quality, person-centred home care services for our clients. This role is office based in our NSW Parrmatta office – Working hours are 9am – 5pm. After you have passed your probation period, you may work from home 1x day each week.
This is a varied, hands-on role for someone who understands government-funded home care, builds trusted relationships and takes genuine ownership of the client experience. You will work closely with clients, families, care professionals and service providers to ensure each client receives responsive, safe and well-coordinated support. this role
Role responsibilities:
Registered Nurses and Enrolled Nurses may also undertake clinical assessments, reviews and high-risk client monitoring within their qualifications and scope of practice.

About the Role
As a Sunnycare Home Care Worker, you will provide support and assistance to elderly clients with Home Care Packages (Level 1–4), helping them live independently in their own homes.
Your duties may include: • Personal care • Medication assistance • Companionship and social activities • Domestic tasks and household support • Meal preparation • Shopping assistance • Transportation to appointments and activities
About You
To be successful in this role, you will have:
• Experience providing home care services to elderly clients and/or NDIS participants
• Professional proficiency in English
• Strong communication and teamwork skills
• A smartphone with internet access
• Current First Aid and CPR certificates
• Current Australian Federal Police Check
• Valid Australian Driver’s Licence
• A reliable and clean registered vehicle with comprehensive insurance
• Certificate III in Aged Care / Individual Support / Home and Community Care
• Australian residency or valid working rights in Australia
