Responsibilities

ABOUT THE ROLE:

Chosen Care Group is currently recruiting Home Care Worker with a minimum of six months experience in delivering Personal Care, and housekeeping tasks.
You need level 1 in Health and Social Care or equivalent, effective communication skills, and prepared to work flexibly to meet our Patients needs in the community.

MAIN DUTIES :

· Assist patient’s with personal care tasks, such as washing, dressing, preparing meals, feeding and toileting.
· Perform light housekeeping tasks, such as dusting, vacuuming, and changing bed clothes.
· Assist in the safe lifting, transferring, repositioning and movement of patients.
· Support all aspects of medication and accompany patient’s to medical appointments.
· Observe, monitor, and record patient’s physical and emotional well-being and promptly report any changes to senior staff.
· Encourage patient’s to participate in social and recreational activities while helping them maintain independence.
· Ensure effective administration and reporting is carried out for every patient you provide care and able to work under minimal supervision.
