To support clients to remain living in their own home through the provision of respite, personal care, domestic assistance and carer support in accordance with care plans and as directed by Care Advisors To provide accurate and timely information about client needs/change in client condition to the Care Advisor Listen to clients and give them a voice, enabling them to choose for themselves Be part of a multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers”. Responsibilities

Provide respite, personal care, and domestic assistance to clients to support them in living independently at home. Communicate client needs and changes in condition to Care Advisors while ensuring clients have a voice in their care.
