Job Details Job Location: Loves Parks, IL 61111 Position Type: Full Time Education Level: High School Salary Range: $21.70 - $21.70 Hourly Travel Percentage: None Job Shift: 1st Shift, 2nd Shift and 3rd Shift Under the supervision of the QIDP, RSD and Administrator, monitors and directs the development, implementation, monitoring, and delivery of all individual services to ensure the active treatment is provided to the individuals of assigned homes. Functions as supervisor on duty for entire facility on a rotating basis. DESCRIPTION OF ESSENTIALDUTIES AND RESPONSIBILITIES: Reliable and prompt attendance as scheduled, demonstrating flexibility as necessary to meet individual, home, and/or facility needs. Directs and monitors correct implementation of Individual Service Plans for assigned individuals. Directs maintenance of records of individuals served, ensuring correct and timely completion of daily data, monthly summaries, and other records maintained in the home. Completes additional reports as assigned. In consultation with individuals served and home team members, ensures ample opportunities for appropriate community activities are provided for all assigned individuals. Continually seeks to develop new activities and resources. Develops and monitors, with input of individuals and team members, a daily routine for the home which ensures active treatment at all times for individuals served in the appropriately sized groupings. Daily routine will clearly reflect choices of individuals served. Reviews injury reports for assigned home, ensuring timely investigations of unknown and significant injuries and follows up as necessary to ensure the safety of the individuals. Monitors team member performance providing timely performance, feedback, prompt evaluations, training and disciplinary action as needed. Works as a partner with QIDP’s ensuring appropriate habilitation planning, implementation of programs and effective communication about individual needs. Supervises team/shift leaders’ performance, providing frequent feedback, prompt evaluations, and discipline as needed. Provides training, direction and support to team/shift leaders in orienting, supervising, monitoring, and training team members. Schedules, conducts and actively participates in regular home meetings. Oversees scheduling of facility/home team members to ensure adequate coverage to meet individual and/or facility needs. Anticipates and plans for special circumstances and functions in full cooperation with all Milestone departments, including personnel and day program, keeping overtime at a minimum. Ensures completion and distribution of meeting minutes in a complete and timely fashion. Requests program funds and individual served personal funds as needed, demonstrating responsible fiscal management. Ensures prompt and correct completion of all documentation as required. Actively participates in IDT/Communication meetings to ensure coordination of all services, maintains a positive working relationship with all departments. Participates in development of policies and procedures related to individual care. Demonstrates knowledge of Department of Human Services, Illinois Department of Public Health Standards and ensures the home is in compliance with all guidelines of authorized inspecting agencies. Reviews and acts upon inspection reports as directed. Actively promotes the maintenance of an exceptionally clean, safe, and pleasant living environment for both individuals served and team members. Assists Maintenance Department and other personnel in the maintenance of a high-quality living environment. Ensures all necessary supplies are available on the home. Ensures all individual needs are recognized, met and documented, including an adequate supply of well-fitting and attractive clothing and personal hygiene, supplies, and possessions. Works effectively with nursing to ensure individual’s health/medical needs are satisfied, including alerting nurses to any health/medical concern, directing staff during any medical emergency, and ensuring prompt emergency intervention when needed. Assigns staff to provide transport to hospital facilities when needed. In compliance with Milestone, Inc.’s philosophy and policies regarding human rights, immediately intervenes on behalf of the individual served, and investigates and/or reports any questions or concerns to the appropriate personnel. Promptly investigates complaints of individuals served, staff concerns, and human rights request for interventions (as assigned) and provides timely adjudication and recommendations. Functions as "supervisor on duty” on a rotating basis, adhering to established policy and procedures, and ensuring the safe functioning of the facility. Completes the prescribed amount of Quality Assurance checks and Program Observations and are completed each month. Meets with Team of Home Coordinators 2x/month to schedule self-including AM/PM SOD coverage leaving no openings in schedule and turn in schedule to Administrator. When the need arises, will function as part of the staff count to work directly with the individuals to ensure their needs are met. Oversee, participate and conduct emergency drills including being an active participant in conjunction with Emergency Preparedness Awareness Training requirements under Public Health regulations. Ensure the completion of staff injury reports as they occur including follow-through necessary initial medical treatment and notifying the AOD and/or Administrator in a timely manner. Performs other duties as assigned. Participates as a trainer in Direct Support Professional training as assigned. This includes, but is not limited to, behavior modification, crisis management, and active treatment. Includes participation in format revisions and follow-up training in the home. Ensuring OJTs are completed by their deadlines. Qualifications QUALIFICATIONS: Prefer minimum age of 21. Prefer minimum of one year's experience in the field. Prefer minimum of one year's experience in supervisory role. Must be approved to drive Milestone vehicles. Must be able to intervene physically in behavioral and health related crisis with individuals served.
