Pediatric experience preferred Job Summary Provides personal care services under the direction of the Registered Nurse or Therapist. The Aide is assigned to specific clients by the Registered Nurse or other appropriate professional who performs services for clients as necessary to maintain their personal comfort and well-being. Reports To: Clinical Supervisor Provides Supervision To: NA – No supervision responsibilities Responsibilities Provide companionship and, if authorized, transportation to clients according to the care plan. Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation/transfers. Report any changes in the client’s condition to the supervisor as outlined in the care plan. Document care provided and maintain accurate records for the Agency. Work collaboratively with the client’s care team and ensure effective communication. Promote client safety by following infection control practices and reporting unsafe conditions. Attend in-service programs to meet compliance and notify supervisors of any educational needs. Qualifications Successful completion of a formal Indiana certification or competency training program or an Agency-administered training program that meets the requirements of Medicare Condition of Participation 42 CFR 484.80. Be at least eighteen (18) years of age. Demonstrated ability to read, write, and follow a written care plan. Good verbal, written, and interpersonal skills. Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Current CPR certification.
