Position Overview: The Home Health Aide is a member of the Courtyards team who assists in meeting the personal care, social, and environmental needs of the Courtyard residents. Essential Duties & Responsibilities: Appropriately assist clients with personal care activities in accordance with the personal services care plan including: Baths (tub, shower, bed bath), mouth-care, denture-care, skin-care, foot-care, ambulation, bathroom privileges (bedpan, commode, toilet), transferring, dressing & undressing activities, grooming activities, and positioning. Appropriately assist with household tasks directly essential to the client's health and in accordance with the plan of care, including: planning and preparing nutritional meals, assisting in feeding client if appropriate, light housekeeping of client's immediate environment, change linen, make bed, laundry of client's clothing & bedding. Count, dispense, and assist residents with medications; complete necessary documentation. Immediately report any changes in resident’s condition, incidents, or accidents, to the Charge person. Accurately measure and record oral, rectal, auxiliary temperatures, pulse, respiration and blood pressure when ordered. Monitors residents, recognizing and reporting any changes in resident condition to the supervisor. Basic Duties & Responsibilities: Assist with medications, may not administer, may remind client, read label for client, bring medications to client, position client. Client must be self-directing. Assists in the orientation and training of new employees. Maintains cordial relationship with clients and their families. All other duties as assigned. Requirements Education Required: Home Health Aide certified. High School Diploma or GED preferred. Ability to read, speak and write English is also required for this position. Experience Required: Prior experience working with the dementia resident preferred, and a genuine interest in the geriatric resident is required. Additional Qualifications: Must be able to communicate with co-workers, residents, and the families of the residents. Dependable, prompt, tactful, patient, a desire to work with geriatric residents. Client-focused at all times, while acting as a pleasant professional is essential.
