The Position: We're looking for passionate Home Health Aides provides personal care and assigned therapy exercises within the patient's home; assists patient in reaching the maximum level of self care. * Provide activities of daily living (ADL) care including bathing, hair care, nail and skin care, and oral hygiene. * Serve as an integral member of the interdisciplinary team. * Recognize and report changes in skin condition to Case Managers. Requirements: * High School Diploma or equivalent. / Minimum age of 18. * Completion of state-approved Nurse Aide Training Program required. * Certified Nurse Aide (CNA) certification with the State of Hawaii preferred. * Must maintain current BLS Certification. * Current and valid 2 Step TB clearance and health review (can be done in-house). Responsibilities
Provide personal care and assigned therapy exercises to patients within their homes to help them reach maximum self-care. Responsibilities include performing activities of daily living and reporting skin condition changes to Case Managers.
