Key responsibilities • Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting • Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan • Prepare meals and assist with feeding as needed • Support medication self-administration through reminders (no independent med administration) • Monitor and report patient condition and changes to the supervising nurse • Maintain a clean, safe, and orderly client environment • Assist with linens and light client laundry as appropriate • Use safety equipment properly (rails, supports, stools, etc.) • Document care provided according to company and regulatory standards • Travel reliably to patient homes and assigned visits across your service area • Complete required in-service training Must-have requirements • Age 18 or older • Active HHA or CNA certification or willingness to obtain certification or 1 year experience is the last 2 years • Willingness to complete competency and skills assessments • Ability to follow written care instructions and document services • Compassion, patience, and professionalism with vulnerable populations • Ability to work independently in home settings • Reliable transportation and ability to travel to client homes • Ability to lift up to 25 pounds • Willingness to complete background screening, and onboarding requirements If you’re ready to make a daily difference in someone’s life — we’d love to meet you.
