JOB OVERVIEW

We are seeking compassionate and dedicated Home Health Aides to provide essential support to individuals in their homes. As a Home Health Aide, you will play a crucial role in enhancing the quality of life for our clients by assisting them with daily activities and personal care. This position requires a strong commitment to patient care, attention to detail, and the ability to work independently while following care plans.

QUALIFICATIONS

  • Previous experience in home health care, assisted living, nursing homes, or long-term care settings is preferred.
  • Knowledge of medical terminology and experience with medical records management is beneficial.
  • CPR certification is required; additional certifications in caregiving or specialized care (e.g., hospice care) are a plus.
  • Ability to perform catheter care and assist with other medical needs as directed by healthcare professionals.
  • Strong observational skills for monitoring patient conditions and recognizing changes that may require intervention.
  • Excellent communication skills to effectively interact with clients, families, and healthcare team members.
  • A caring demeanor with a passion for helping others in their home environment.

Responsibilities
  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
  • Provide companionship and emotional support to clients, fostering a positive environment.
  • Monitor and document vital signs, changes in client condition, and report any concerns to the supervising nurse or healthcare provider.
  • Administer medication as prescribed and ensure compliance with medication schedules.
  • Assist with meal preparation and ensure proper nutrition for clients.
  • Perform light housekeeping duties including cleaning, laundry, and maintaining a safe living environment.
  • Utilize EMR/EHR systems for accurate medical documentation and record keeping.
  • Provide specialized care for clients with conditions such as Alzheimer’s, dementia, or developmental disabilities.
  • Operate medical equipment such as Hoyer lifts when necessary for client mobility assistance.
  • Ensure adherence to HIPAA regulations while handling patient information.
