We work closely with clients and families to ensure care plans are responsive, appropriate, and clinically sound. Your Role As an RN in this role, you will focus on clinical assessments, care coordination, and monitoring client progress, rather than routine bedside nursing tasks. Key Responsibilities: Conduct initial client intake assessments in the home or community Complete monthly follow-up visits to reassess client needs and progress Develop, review, and update individualized care plans Collaborate with Care Coordinators and caregiving staff to ensure care aligns with clinical recommendations Provide clinical guidance and support to the care team Maintain accurate, detailed, and timely documentation Identify changes in client condition and recommend appropriate interventions Ensure compliance with infection control standards and professional practice guidelines What We’re Looking For Licensed Registered Nurse in British Columbia (in good standing) Current CPR certification Strong assessment and care planning skills Excellent communication and interpersonal abilities Comfortable working independently in the community Strong organizational and documentation skills Negative TB test or chest X-ray Valid driver’s license and access to a vehicle This is a great role for an RN/LPN who prefers a consultative, assessment-focused position with flexibility and meaningful client interaction—without the physical demands of shift-based bedside care.
