At Choices Home Health, we’re looking for a hungry, motivated Sales Liaison who thrives in the field, knows the local market, and is ready to own their territory and drive real growth. 🔥 WHAT YOU’LL OWN * Your territory — build, grow, and dominate it * Your referral network — physicians, hospitals, case managers, SNFs, and community partners * Your results — drive consistent patient admissions and exceed monthly goals ---------------------------------------- 💼 WHAT YOU’LL DO * Develop and maintain high-value referral relationships that consistently generate business * Position Choices Home Health as the go-to provider through strategic, in-person outreach * Identify new accounts, open doors, and close referral opportunities * Collaborate with clinical teams to ensure smooth patient transitions and exceptional service delivery * Stay ahead of market trends and competitor activity * Execute targeted outreach and marketing strategies that drive admissions * Consistently meet or exceed sales/admission goals Requirements * Minimum of 2 years home health or hospice sales experience preferred * CRM Software experience preferred * Valid Driver’s License * Valid Auto Insurance * HomeCare HomeBase experience preferred Benefits and Perks * Medical, Dental, Vision Insurance * 401k You’re eligible after 3 months of service/The plan is 100% fully vested immediately/Choice Health At Home contributes 100% of the first 3% you contribute each pay period * Health Savings Account * Life Insurance * Short & Long Term Disability Insurance * Paid Time Off and Paid Holidays * Company vehicle upon availability after 90 days of full time employment and all the terms and conditions set by the Fleet Car team are met.
