FROM THE HEART DIRECT CARE WORKER JOB DESCRIPTION

· From the Heart requires a Pennsylvania State Police Criminal Background check. Applicants who have not lived in the state of PA for the last 2 years upon hire will be required to have an FBI clearance done. A Childline clearance is required if there is a child living in the home.
· Also required is one of the following: a two-step TB test, a chest x-ray specific to rule out TB, or a QuantiFERON gold tb test. All of the above required testing is at the expense of the applicant.
· Additional requirements:
o Successful completion of a competency test with a passing rate of 70% or better, or proof of a prior successful test within the last year that covers all of the required state topics; or a CNA certificate showing completion of a state approved CNA course
o Compassion, empathy, and a genuine desire to help others
o Strong communication and interpersonal skills, including the ability to clearly communicate and actively listen
o Basic understanding of personal hygiene and healthcare practices
o Reliable, punctual, and trustworthy
o Physical stamina and the ability to perform caregiving duties effectively
o Willingness to enforce health and safety standards
o Minimum high school diploma or GED
o Available for variable shift hours
o Have reliable transportation to and from client homes (not all clients are on bus lines)
o Valid driver’s license or valid PA ID
o Proof of PA residency or FBI clearance
The following outline describes the primary responsibilities expected of a Direct Care Worker (DCW) employed by From the Heart. Additional duties may be required based on client need and will be notated in the client Service Authorization.

ESSENTIAL JOB FUNCTIONS:

  • Assist with Personal Care – Dressing, Grooming, Bathing
  • ADL/IADL functions as noted on Service Authorization
  • Assist with walking and light exercise
  • Plan and prepare meals, followed by clean up
  • Make beds and change linens as needed
  • Light housekeeping - includes dusting, vacuuming and bathrooms
  • Laundry
  • Take out trash
  • Medication Reminders
  • Socialization and activities
  • Maintain Calendar and organize mail
  • Run Errands/Shopping
  • Accompany to appointments
  • Arrange Transportation and appointments
  • Report any significant client condition change to the Agency office
    At the discretion of the supervisor, you may be required to perform additional duties not listed in your job description, as needed.
