Description We are currently seeking a Full-Time Home Health RN Case Manager to join our skilled home health team. In this role, you will manage the comprehensive care of patients in their homes, providing skilled nursing care, education, and coordination of services. This is an excellent opportunity for an RN who enjoys building long-term relationships with patients and families, working independently in the field, and being a vital part of a collaborative healthcare team. Schedule: M-F 8-5. On Call Rotation Key Responsibilities: Perform comprehensive assessments of patients in their homes and develop individualized care plans. Provide skilled nursing care, including medication management, wound care, and chronic disease management. Collaborate with physicians, therapists, social workers, and other members of the care team to ensure optimal patient outcomes. Educate patients and caregivers on disease management, treatment plans, and self-care techniques. Monitor and document patient progress, adjusting care plans as needed. Communicate regularly with the clinical team to ensure coordinated care. Participate in case conferences and interdisciplinary meetings. Travel to patient homes throughout the Cincinnati area. Requirements Active RN license in the State of Ohio (required). Minimum of 2 years of nursing experience; home health experience strongly preferred. Strong clinical assessment, critical thinking, and organizational skills. Compassionate, patient-focused approach to care. Excellent communication and interpersonal skills. Ability to work independently and manage a flexible schedule. Valid driver’s license, reliable transportation, and current auto insurance. Typical Working Conditions: The majority of work is performed in providing patient care services in the patient’s home. Exposure to communicable diseases is common.
