POSITION SUMMARY

The home health care clinical coordinator plays a key role in supporting the coordination and delivery of high-quality, patient-centered care in the home setting. This position is approximately 70% office-based and 30% field-based as a treating clinician. The role includes participating in the on-call rotation and assisting with direct care as needed. This is a non-supervisory position, focused on collaboration, coordination, and compliance—not direct staff management.

EDUCATION, LICENSURE & EXPERIENCE

  • Graduate of an accredited nursing program
  • Current Registered Nurse (RN) license in the State of Nebraska; ongoing CEUs required
  • CPR certification required (or willingness to obtain)
  • Minimum of two (2) years of home health experience required
  • Experience in long-term care or acute care preferred

QUALIFICATIONS

  • Must meet all qualifications to practice as an RN in Nebraska

  • Comfortable working independently in both clinical and administrative environments

SKILLS & COMPETENCIES

  • Strong organizational and communication skills
  • Proficient with clinical software and general office systems
  • Ability to work independently, prioritize tasks, and remain calm in high-pressure situations
  • Familiarity with general office procedures (phones, faxing, copying, etc.)

PHYSICAL REQUIREMENTS

  • Must be able to stand, walk, bend, kneel, and occasionally lift up to 80 lbs
  • Adequate vision, hearing, and manual dexterity required for clinical care
