Overview We are seeking a Supervisor of Clinical Services to oversee our interdisciplinary clinical team in Home Health Care. This leadership role involves planning, organizing, and guiding staff to deliver effective, patient-centered care. The Clinical Manager supervises, supports, and educates staff directly in the field while ensuring compliance and quality. Lead an interdisciplinary team managing a patient census of ~200. Ensure adequate coverage for weekdays, weekends, and holidays. Interview, hire, and onboard salaried, per diem, and contract staff. Support and guide field staff through patient calls, triage, and follow-up. Oversee patient assessments, care coordination, and plan of care updates. Collaborate with QAPI and Education to support staff growth and OASIS documentation quality. Hold clinicians accountable for agency standards, timely documentation, and productivity goals. Provide role modeling, direct observation, and performance evaluations. Participate in policy design, quality improvement, and compliance activities. Requirements: Licensed Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) in NY Bachelor’s degree in Nursing, Physical Therapy, or Occupational Therapy At least 2 years of recent Medicare Home Health experience Some management experience in any healthcare setting U.S. Citizen or Green Card holder (no exceptions) COVID-19 vaccine required To Apply, Please Include Answers to the Following: 1.
