Description
As a Home Health LVN/LPN, you’ll practice at the top of your license—delivering skilled, one-on-one care in patients’ homes. You’ll have the autonomy to use your clinical judgment while making a real impact on patient outcomes and independence.  Monitor assigned cases to ensure care is delivered per the Plan of Care, including tracking patient progress, identifying changes in condition, and reporting updates to the RN/physician.  Independence with support – Work in the field while staying connected to a strong team What You’ll Do * Partner with the RN to assess and implement individualized care plans * Provide skilled nursing care, including medications and wound care * Educate patients and caregivers on treatment and disease management * Communicate effectively with the care team to ensure continuity and quality * Monitor assigned caseload to ensure continuity, quality, and compliance.
