Essentials Duties & Responsibilities: * Assist a Nurse Case Manager in providing direct patient care. * Initiate communication with team members, HHAs, and other healthcare providers to facilitate optimal care. * Instruct primary caregivers, patients, volunteers, and employed caregivers in care provision based on the Plan of Care as requested by the Nurse Case Manager or Team Leader. * Perform nursing procedures within the scope of a Licensed Vocational Nurse. * Maintain current documentation in a computerized clinical documentation system according to organization policies, procedures, and applicable laws and regulations. Required Qualifications: * Graduate of an approved and certified LVN program. * Current and unencumbered license to practice as an LVN in the state. * Ability to make quick judgments and work independently. * Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines. * At least one year of LVN clinical experience; home health experience preferred. * Ability to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication. * LVN / LPN with current licensure to practice in the state * Strong communication and organizational skills * Basic knowledge of computer use or willingness to learn Skills & Attributes: * Strong critical thinking skills when working independently in the field. * Excellent interpersonal skills to build rapport with diverse patient populations. * High level of organization and time management to navigate a daily route. Nice to have: * Experience in acute care and working in a home environment. * Wound care experience in wound care/vac, post-surgical care, catheterization. * Experience with KanTime EMR. Responsibilities
