T As a Home Health RN, you will provide high-quality, patient-centered care in the comfort of patients’ homes while helping patients achieve independence, improved outcomes, and enhanced quality of life. WHAT YOU’LL DO * Complete initial and ongoing comprehensive patient assessments including physical, functional, psychosocial, and environmental needs * Evaluate eligibility and establish appropriate levels of home health care services * Develop, implement, and update individualized plans of care * Educate patients and families on disease management, self-care techniques, medications, nutrition, and safety * Determine frequency and scope of services based on patient acuity and caregiver support * Perform preventative and rehabilitative nursing interventions * Maintain infection prevention and standard precaution practices * Supervise and provide clinical guidance to Home Health Aides and LPN/LVNs * Document patient progress and clinical outcomes accurately and timely * Communicate changes in patient condition to physicians and interdisciplinary team members * Ensure compliance with payer requirements and quality standards QUALIFICATIONS * Current RN License * Valid Driver’s License * Current CPR Certification * Valid Auto Insurance * Minimum 2 years of Home Health RN experience strongly preferred * HomeCare HomeBase experience preferred WHY JOIN CHOICE HEALTH AT HOME? Responsibilities
