SUMMARY

We are seeking a strong, solutions-oriented Lead Care Coordinator who excels at driving team performance, proactively identifying operational gaps, and fostering a culture of accountability and continuous improvement. This role is critical in guiding our care coordination and intake teams to consistently meet targets and deliver exceptional patient care. The ideal candidate will exhibit a strong leadership presence, emotional intelligence, and the ability to hold others accountable while maintaining team morale.

Required

  • Proven leadership experience in healthcare operations or care coordination.
  • Strong organizational, analytical, and problem-solving skills.
  • Proactive work ethic and commitment to continuous personal and professional growth.
  • High emotional intelligence (EQ), including the ability to:

o Address underperformance constructively and timely.
o Recognize and respond to team members’ emotional needs.

  • Excellent communication skills—clear, assertive, and empathetic.
  • Tech-savvy and adaptable to Microsoft Office Suite, EMRs, and communication platforms.
  • Calm, flexible, and effective in a fast-paced environment.
  • Fluent in English (written and verbal).
  • Must be available to work Pacific Time Zone (9AM–6PM).
  • Ability to hold team members accountable for performance and documentation compliance.
  • Experience with conducting coaching, performance reviews, and corrective actions.
  • Collaborative mindset to work cross-functionally with Sales, Clinical, and QA teams.
  • Able to address and resolve escalated patient or team concerns promptly and effectively.

We’re looking for a candidate with 2+ years of leadership experience in the medical field ONLY. Must have a strong background in clinical operations or patient care coordination.
