Description About the Role: At Corus Home Health, we’re looking for a compassionate and licensed LPN to join our close-knit home health team in Albuquerque, NM. This is a field-based role offering autonomy, flexibility, and the chance to make meaningful, one-on-one connections with patients where they feel most comfortable — at home. Key Responsibilities: Deliver skilled nursing care under the supervision of a registered nurse (RN) and in accordance with patient care plans Monitor patient conditions and report changes to the supervising clinician or physician Perform treatments and administer medications as prescribed Educate patients and caregivers on managing illness and treatment plans Document all care activities accurately and promptly using EMR systems Collaborate with interdisciplinary team members to support patient goals Travel locally to provide care to patients throughout the Albuquerque area Participate in team meetings, ongoing education, and quality improvement initiatives Requirements What We’re Looking For: Current LPN license in the state of New Mexico Valid driver’s license, auto insurance, and reliable transportation Experience in home health or a similar clinical setting (preferred but not required) Strong communication and time management skills Comfortable working independently in patient homes Ability to adapt care to a variety of home environments and needs Commitment to compassionate, high-quality patient care Tech-savvy enough to document care using mobile EMR systems Why Join Us: At Corus, you’re more than a clinician — you’re a vital part of a mission-driven team that’s committed to bringing out the courage in others.
