As a Home Health LPN Field Nurse, you will follow plans of care, provide treatments, and collaborate with members of the team to meet positive patient care outcomes. Responsibilities

The Home Health LPN Field Nurse will follow established plans of care, administer necessary treatments, and collaborate with the healthcare team to achieve positive patient outcomes. Primary duties include coordinating home health services, reporting patient needs to the RN or Clinical Manager, assessing patients, providing education, and implementing the RN-initiated plan of care.
